Dr. Brent’s ADHD assessment process in 3 steps
Intake Interview with Dr. Brent (50 minutes on Zoom)
Complete a historical review of your executive functioning challenges and strengths.
Review functioning impairments in the different domains in your life that ADHD may impact.
Provide you with online ADHD and Medical assessments to complete.
2. ADHD and Strengths Assessment Forms (online)

Barkley Functional Impairment Scale
PHQ-9 (Patient Health Questionnaire)
GAD-7 (General Anxiety Disorder)
Medical assessment form
Sleep Assessment

3. ADHD Assessment Results review and discussion with Dr. Brent (30 minutes)
This step is completed with Dr. Brent Hogarth over Zoom.
4. Receive your ADHD Diagnosis Report
You will receive your result within 2 weeks of completing your assessment report.
5. Focus and Flow Coaching plan with Dr. Brent (OPTIONAL)
Beyond the ADHD Assessment report, you may option in to receiving a comprehensive coaching program to train up your executive functioning skills. This includes a formal treatment plan and up to 8 ADHD Coaching sessions with Dr. Brent Hogarth.
